Home Visit Fee (08:00 - 18:00)= $25 for the first 15 miles from Salisbury MD and $ 2 per mile afterward
Pre-Euthanasia Sedation = $30-45
Euthanasia Service = $245-280
Self Cremation = $0; I agree to Proper Disposal Consent stated below.
Lotus Vet Private Cremation Service = $360
Euthanasia Service after 6 pm, weekends, or holidays = + $110
Consent Compassionate At-Home Euthanasia
I certify that I am the owner or authorized agent for the owner of the pet described above and that, to the best of my knowledge, my pet
1) has not been exposed to any animal that is likely to have rabies,
2) has not bitten any person or animal within the past ten days, and
3) is current on its rabies vaccination.
I consent to the euthanasia of this pet and the Burial & Cremation of the body as agreed upon between the attending veterinarian and me.
Proper Disposal

Proper Disposal: I agreed that Incineration/Cremation, if available, is the preferred method of disposal.
2 I agreed to Immediate deep burial of least 3-4 feet of cover with additional amounts or types of exclusionary cover will prevent access to scavengers.
3 I agreed to properly address the required distance above groundwater table, appropriate soil type, and permitted locations for burial pits.
4 I agreed to Double bagging with “heavy duty” sacs and clear labeling of poison carcasses
5 I agreed to Local landfill regulations and management practices
